Police Request for Information Form

This form is to be filled out by a member of the Police Force when requesting the release of personal rider information from Beam Mobility in regards to any incidents involving a BeamRider or Beam E-Vehicle. The more detailed information you can provide about the incident, the faster and more effectively we can assist you with your inquiries.
